  2. Your contact information, including your name, address, phone number, and email address.
  3. A statement under penalty of perjury that you have a good faith belief that the material was removed or disabled as a result of mistake or misidentification.
  4. Your physical or electronic signature.

4. Removal of Infringing Content

Upon receiving a valid DMCA takedown notice, BloggyMy.com will promptly remove or disable access to the infringing content. We will notify the user who posted the content about the removal.

5. Repeat Infringers

In accordance with the DMCA, BloggyMy.com may terminate the accounts of users who are found to be repeat infringers.
